WHO WE ARE: WE ARE YOUR PATIENT-CENTERED MEDICAL HOME

The American Academy of Pediatrics (AAP) promotes access to quality Medical Homes for all children and youth. A Medical Home is a comprehensive approach to healthcare that helps families partner with their child’s pediatrician to ensure excellent care. This includes routine exams and “sick visits” as well as health education, family support, and anything else that may improve your child’s overall health. Our pediatric practice has been recognized as one of the first NCQA (National Committee for Quality Assurance)-certified Medical Homes in Norwalk, CT.

At TLC Pediatrics, our primary focus is to provide comprehensive, personalized care to our patients and their families in a compassionate and welcoming environment. Our practice is culturally diverse and we are sensitive to the needs of many different cultural backgrounds. As a Medical Home, our pediatricians will:

- Diagnose and treat your child when he/she is sick or well

- Talk with you about any testing or treatment that your child needs

- Review all laboratory and procedure results within 48 hours when applicable

- Help you plan your child’s care and set goals for the future

- Help you coordinate care with other health care providers when needed

Dear Patient:

The providers at TLC strongly encourage you and your children ages 5 and up to get vaccinated against COVID-19. The vaccine has been proven to be safe and effective, and in children 5-11 prevents 90% of all symptomatic COVID cases. Any side effects in this age group have been mild, and less common than the side effects in adults and older children. If you have any questions or hesitations about the vaccine for your child(ren) please do not hesitate to reach out! We’re here to help!

We are still working hard to get the vaccine at TLC, but we don’t know yet when this might happen. However, we do encourage you to get your children vaccinated as soon as possible! You can schedule an appointment right away by visiting https://covidvaccinefinder.ct.gov and selecting the appropriate vaccine (Pfizer 5-11)

Additionally, you can go to the individual websites of the following organizations to find vaccine appointments:

Yale Health System https://covidvaccine.ynhh.org/

Norwalk Community Health Center https://www.norwalkchc.org/

Walgreens https://www.walgreens.com/topic/promotion/covid-vaccine.jsp

CVS https://www.cvs.com/immunizations/covid-19-vaccine/kids

Remember, your child can also receive the flu shot at the same time!
